Also, please be aware that all submitted forms must be properly signed or approved. In place of a wet signature on the form the supervisor can send an email indicating they approve the form and state the client name of whom they are approving. You can attach that email to this submission. Attachments Please be sure this CUNYfirst form is properly filled out and is signed by the requestor. The requestor can use the "Confidentiality Agreement Code" while the immediate Manager can approve via this form using the Approval and Comments box. Follow those instructions carefully.
