Dependent Economic Eligibility Form (2024 - 2025)
For SEEK scholars whose tax information is filed as a dependent under a parent or legal guardian.
Independent Economic Eligibility Form (2024 - 2025)
For SEEK scholars that file independently or are not claimed as a dependent on someone elses tax returns.
CUNY Immunization Record Form & Meningitis Fact Sheet
This form is required before registration.
CUNY Special Programs Transfer Request Form
This form is to be completed by the college/program officials only after the scholar has submitted a Transfer Application. Only one form needs to be filled out for each scholar even if they are applying to multiple colleges.
If you are not returning for the upcoming semester you must have a Leave of Absence form completed before the next term.
Permission for Part-Time Attendance
SEEK scholars must get special permission from their SEEK counselor and SEEK Director before they register for less than 12 credits (part-time). Save this file as a PDF when submitting.
Permission to Attend Evening and/or Weekend Classes
SEEK scholars must get special permission from their SEEK counselor and SEEK Director before they register for courses that begin after 5pm and courses that meet on Saturday and/or Sunday.
As a SEEK scholar with a declared major you must seek advisement from your major advisor before you can register. The form must be completed along with the advisor (in your major) and brought back to the SEEK counselor for academic advisement block removal.
In order to maintain eligibility in The Percy Ellis Sutton SEEK Program, SEEK scholars must complete and adhere to the guidelines in the following contracts:
